Community Care Cooperative (C3) is a 501(c)(3) nonprofit, an FQHC-led ACO and MSO serving Federally Qualified Health Centers. Our mission is to leverage the collective strengths of FQHCs to improve the health and wellness of the communities we serve. We negotiate value-based payer contracts, manage contract operations (including compliance, financial management, and population health), and operate a range of services nationwide.

Job Summary: The Program Manager, Quality provides support to C3’s Quality Program, including development, management, and implementation of the annual quality plan; monitoring quality performance on value-based contracts; providing expertise on quality management and improvement; identifying improvement opportunities; working cross-functionally with health centers; and supporting C3 health center-facing teams to facilitate improvement. This position reports to the Director of Quality and is based in Boston, MA with a hybrid work arrangement.
