Ennoble Care is seeking a Memory Care Program Manager to lead the day-to-day operations of the GUIDE Memory Care Program in Marlton, New Jersey, on-site. This role owns the program's administrative and CMS compliance functions, and serves as the day-to-day operational and escalation point for Memory Care Coordinators, while personally carrying a portion of the GUIDE patient panel. Key responsibilities include managing clinical workflows, scheduling, and operational infrastructure in line with CMS guidelines; supervising and coaching Memory Care Coordinators; coordinating non-clinical assessments, care plans, caregiver education, and respite services for assigned patients; ensuring accurate documentation and billing in accordance with DCMP standards; overseeing respite administration, beneficiary enrollment, and CMS reporting; developing dashboards and quality improvement initiatives; and building partnerships with home-based providers and community organizations. Qualifications include a Bachelor's or Master's degree in nursing, healthcare administration, social work, public health, gerontology, or related field, with 5+ years in dementia care or care coordination and leadership experience; active RN or LPN license preferred.
