Responsibilities

Educate healthcare professionals, patients, and families about home care services. Develop and maintain relationships with key referral sources, including hospitals, nursing homes, and physician offices, to generate leads. Coordinate admissions with the team, including obtaining required documentation for referrals and admissions. Conduct on-site visits to assess patient needs and identify barriers to care. Maintain accurate records of patient interactions and referrals in the CRM. Achieve admissions, product, and payor-mix goals. Meet short- and long-term target account goals. Document the minimum expected daily sales calls in the CRM (pre- and post-call notes). Maintain an up-to-date record of targeted referral sources in the territory in the CRM. Attend community events to promote Amedisys. Respond to customer complaints in a timely manner. Perform other duties as assigned.
