The focus includes CMS Transforming Episode Accountability Model (TEAM), bundled payments, transitional care, and value-based reimbursement initiatives, supporting episode performance, transitions of care, post-acute network effectiveness, and measurable improvements in quality, utilization, patient outcomes, and patient experience. Responsibilities include leading performance improvement through clinical expertise and collaboration with clinical and non-clinical stakeholders, adopting AI-enabled tools, analytics platforms, Epic enhancements, and emerging technologies to improve outreach, care coordination, documentation, and program performance.
