Program Manager, Enhanced Care Management (ECM)

Harmonium is seeking a part-time ECM Program Manager to lead the Enhanced Care Management program serving individuals and families with complex medical, behavioral health, housing, and social needs. The role may expand to full-time as the organization grows. The ECM Program Manager will supervise care coordination staff and oversee outreach, engagement, care planning, and service delivery to ensure high-quality, trauma-informed, and culturally responsive services aligned with Wellness Collaborative requirements.

Key responsibilities: - Develop, implement, and maintain ECM policies and procedures; ensure staff adherence. - Recruit direct reports and participate in interviews for staff, interns, and volunteers. - Oversee ECM services in alignment with Wellness Collaborative requirements; provide leadership to ECM staff including Lead Care Managers and Housing Navigation staff. - Attend supervisor and collaboration meetings with San Diego Wellness Collaborative. - Ensure timely, compliant documentation across assessments, care plans, progress notes, referrals, and related ECM documentation. - Review and approve individualized care plans; support outreach for high-need populations. - Develop and maintain partnerships with community organizations, healthcare providers, school districts, housing agencies, behavioral health systems, managed care plans, and other stakeholders. - Represent Harmonium in community collaboratives, health plan meetings, county initiatives, and care coordination m
